       For the person who need assistance with activities of daily living,
Seniors Club sends to your home a trained Personal Care Attendant or
                          Certified Nursing Assistant to help you.


                            Personal Care Services Include Assistance With:

                                      Bathing                                 Meal Prepartion
                                      Grooming                        Light Housekeeping
                                      Toileting                             Grocery Shopping
                                      Dressing                                  Socialization &
                                      Eating Assistance                         Supervision
                                      Ambulation                  Medication Reminders

                 Assistance when going to the doctor or other appointments

          All In-Home Attendants must pass a State-wide Background Check

       Attendants are monitored and observed at least every two weeks by an
                                                    Office Supervisor

                                 Registered Nurse oversight and supervision
                            Daily Documentation of services that are provided
